What the Page Numbers Stand for on DHS:

• Page 1 – Traditional Medicaid (NHIC), use if there is no HMO Medicaid plan
• Type Prog – if the number is 13, this is an SSI Recipient. Program will override all HMO Medicaid Plans except the Star Plus Plan.
• QMB – If there is a Q under QMB the patient may have Medicare. It is important to check the number listed above under SSCN and if applicable load Medicare primary and FSC 102 Medicaid secondary. Do not use any HMO Medicaid product when Medicare is primary, al claims will go to NHIC Medicaid.
• Page 3 – Commercial Insurance Plans if applicable
• Page 6 – HMO Medicaid Plans – the HMO plans override Traditional Medicaid
• LTC – If you see LTC (Long Term Care) by the PCP’s name, the patient has Medicare. Check and load Medicare. If the patient does have Medicare, do not load any HMO Medicaid product. All Medicaid claims will go to NHIC, use the secondary FSC 102.
• Medicaid recipients have the options to change their plans every 30 days. You have to be mindful of this and realize that many changes occur and have to be addressed. Therefore, do not load Medicaid plan and leave another plan open with no term date. Make sure the account has accurate insurance information and everything is uniformed.
